Beneficiary Reminder

Have you checked your beneficiary information?

You may need to if you:

  • Have experienced a life event such as a birth or adoption of a child, marriage, death in the family, and/or a divorce/annulment.
  • Made a change to your tax forms.
  • Have had a change in financial situations.
  • Do not have someone already listed.



You can adjust your beneficiary information in these three places:


1. Life Insurance Policies

Please visit the Employee Self-Service (ESS) app, click benefits, and then click manage beneficiaries. Any changes you make to your beneficiary information for life insurance will become effective immediately.

2. Retirement Benefits

Visit Fidelity’s website for instructions.

3. Health Spending Account (HSA)

Visit HSA’s Bank website for instructions.
